When the person with the Facility Administrator (FA) role leaves a facility, the FA role needs to be reassigned to another user. There are two ways to do it.
Before leaving, the current FA can log in reassign the role to another active user at the facility. If the current FA is no longer available, then the facility would need to reassign the role to another user – either an existing active user or a person who does not yet have access to NHSN.
Method 1: NHSN now has a new process for reassigning the NHSN Facility Administrator (FA) role. This process is now streamlined by using a web-based form which can be accessed here: https://www.cdc.gov/nhsn/
Method 2: The facility should complete and have facility leadership sign a reassignment letter using the attached FA Reassignment Template and email it to: email@example.com. (the facility would need to scan and email it or take a photo with their phone and email it)
After CDC receives the reassignment letter and the role has been assigned to an existing user, that user will instantly be able to perform administrator tasks in NHSN for that facility.
After CDC receives the reassignment letter and the role has been assigned to a new user, the user will receive a ‘Welcome to NHSN’ email that will have instructions for him/her to start the process of gaining access to NHSN.
