1. Log into EQRS with your HARP Account.
2. Click on 'Change Organization' to switch from EQRS to QIP
If you do not have the QIP option: apply for access following these instructions: https://help.esrd.ipro.org/a/solutions/articles/9000188457
3. Select your facility
4. Select QIP Scores in the top Left
View your QIP score, Reports, and Feedback.
Submit Preview Period Inquiries (and monitor CMS responses to them) below.
