When the person with the Facility Administrator (FA) role leaves a facility, the FA role needs to be reassigned to another user.  Before leaving, the current FA can log in reassign the role to another active user at the facility. If the current FA is no longer available, then the facility would need to use the Change NHSN Facility Administrator web-based form.

After CDC receives the reassignment letter and the role has been assigned to a new user, the user will receive a ‘Welcome to NHSN’ email that will have instructions for him/her to start the process of gaining access to NHSN.
