CMS requires that facilities notify the ESRD Network about any:
- Change in hours and/or services and/or shifts
- Temporary closure
- Permanent closure
The Network will work with you to ensure that all patients are transferred to other facilities and that your data is closed out of EQRS, the ESRD Quality Data Reporting System.
- Submit a copy of the Closure Letter on corporate letterhead (that includes the official Closure Date) through the Customer Support Request Portal.
- Make sure all patients are discharged from the facility prior to the Closure Date.
