EQRS Facility Services, Facility Shifts, Address Update

EQRS Facility Information is important to maintain so that the Network can correctly assign QIA Goals and IPRO Learn activities. According to the CMS conditions for coverage, facilities are required to notify the state survey agency of the move.

If your facility is CLOSING Temporarily or PERMANENTLY submit this Form:

To update facility information in EQRS:

• Log into EQRS https://eqrs.cms.gov/globalapp/
  • Go to Facilities Tab and select Search Facilities
  • Search by Name or CCN

• Facilities CAN update: Hours/Days Open.

• Facilities CANNOT update: 
  • Certified Services
  • Address
  • Phone/Fax
  • Ownership

• SUBMIT a document on CMS or Corporate letterhead that indicates the correct information and Dates and the Network will update EQRS: https://help.esrd.ipro.org/support/tickets/new.
